What Does Attribution Mean?

What does this mean?

When you are attributed to a healthcare company by your insurance company, the insurance company believes that the healthcare company is your primary care care provider.  As such, we are assigned to look after your health.  In addition to caring for your urgent medical needs, we have been asked to help coordinate your preventive care and ensure there are no gaps in your overall healthcare.

 

What if I don’t want you to be my primary care provider?

In many cases, if you visited us and not your primary care doctor in the last year or so, your insurance company may have assigned us as your PCP. If this information is incorrect, please contact your insurance company using the phone number on the back of your insurance card and request that they update your PCP designation.

 

What’s next

To help us provide the best possible care, Emory has enlisted Guide Health to assist with reaching out to you to coordinate important care issues or gaps. You may receive phone calls or communications from Guide Health or our Care Coordination Team regarding your care. Please connect with these representatives, as they are a direct extension of our clinical team. They may reach out for care including:

  • Preventive Screenings: Such as mammograms and colorectal cancer screenings.
  • Chronic Care Management: Monitoring Hemoglobin A1C levels, blood pressure, medication adherence.
  • Medication Adherence: Ensuring your prescriptions are working effectively for you.

If you have any questions or concerns regarding your care or this initiative, please contact the PIC Care Coordination Team directly at 678-273-7117.  Also, if you are in need of an annual exam, please select a location from the menu above and schedule an annual physical from the visit reasons. These can be set up during extended hours and weekend.
